Occupational Therapist
Position Purpose
The Occupational Therapist (OT) plans and provides care for patients in their homes under physician's orders and with participation by the family, nurse, or other appropriate persons, as necessary. All functions shall be performed in accordance with the established policies and procedures of CareMate Home Health Care, Inc. The Occupational Therapist reports directly to the nursing supervisor.
Professional / Personal Qualifications
1. Be currently registered with the State of Minnesota as an Occupational Therapist.
2. Have at least one (1) year experience in an acute care setting or equivalent experience.
3. Possess and maintain good physical and mental health, including yearly negative Mantoux or negative chest x-ray every year.
4. Demonstrate professional behavior and appearance.
5. Be a U.S. citizen, or have evidence of a valid alien work permit.
6. Ability to exercise independent judgment.
7. Knowledge of principles and practices of Occupational Therapy.
8. Knowledge of CareMate Home Health Care, Inc. policies and procedures.
9. Ability to improvise in an unstructured home setting.
Specific Functions / Responsibilities
1. Assists the physician in evaluating and developing the plan of care and establishing goals based on the patient's current level of functioning and potential for improvement.
2. Guides and instructs the patient in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
3. Establishes household management routines
4. Designs and / or orders appropriate adaptive / assistive devices and / or splints to promote optimal level of functioning.
5. Records and reports to the physician the patient's reaction to the therapy program or any changes; makes changes, addition, and / or deletions to the patient's clinical records within seven (7) days.
6. Instructs patients and their families in activities of daily living.
7. Instructs the family in the patient's total occupational therapy program.
8. Instructs other health team members regarding occupational therapy for specific patients.
9. Assesses the patient's progress towards stated goals and revises the care plan accordingly.
10. Attends team meetings and care conferences as deemed necessary.
11. Participates in staff development activities.
12. Assists the staff nurse in development of the patient's care plan.
13. Serves on the Chart Audit Review Committee.
